Case Management is central point of contact for NDIS participants with OAK service enquiries.

Our specialist team can provide information about services and work with you to create an individually tailored support package in a transparent and accountable way.

Case management will work with you to:

  • Create and administer your plan
  • Review your plans, objectives and goals
  • Identify what supports are required and make the appropriate referrals
  • Assist you to enter and exit services
