OAK Case Management

This is a new concept within OAK Tasmania and to be truly effective and responsive to the NDIS the Case management Service will sit as a central point of contact acting on behalf of the other OAK Tasmania Divisions, to provide the following functions:

  • CasePoint of contact for all OAK Tasmania service enquiries
  • Marketing of internal services and providing information to potential customers
  • Intake assessment of new potential customers to OAK Tasmania Services
  • Individual Participant Planning (IPP) for person centred goals and objectives
  • Coordinating the IPP process including reviews of objectives and goals
  • Identification of individual supports required internally and referral externally as required
  • Reporting on service delivery & Statistics relating to participant outcomes
  • Administration of the Iplanit System including establishment and maintenance of user accounts and authorities
  • Coordinate of transition or exit process for customers wishing to leave the service or move to another area of the service

Establishing a Case Management Service will create a more accountable, transparent and person centred service delivery model within OAK as well as providing an easy to access service for our customer base.

The Case Management Services is located at 6B Lampton Avenue, Derwent Park.
